Whether recovering from an accident, surgery, stroke, or dealing with a long-term health condition, our carers are committed to helping you meet your rehabilitation goals with dignity and respect.
What is rehabilitation care?
Rehabilitation care refers to a comprehensive plan of support designed to help individuals recover and regain independence after an illness, injury or medical procedure. It can involve physical therapy, occupational therapy, speech therapy, and psychological support. The goal is to improve physical function, reduce pain, promote healing, and address the emotional challenges that often accompany recovery.
Rehabilitation care at home allows individuals to progress in their recovery journey without the stress of being away from loved ones or in unfamiliar environments. It also offers a more flexible and personalised approach to care, with carers providing one-to-one support tailored to the unique needs of each individual.

What does our rehabilitation care include?
At Consultus Care and Nursing, our rehabilitation care is designed to be fully personalised, ensuring your loved one receives the support they need to recover effectively and comfortably at home. Our experienced live-in carers work closely with healthcare professionals to follow rehabilitation programs, while also offering compassionate, one-to-one care that addresses both physical and emotional well-being.
Here's what our rehabilitation care typically includes:
Personalised care plans: Each rehabilitation journey is unique, which is why we create tailored care plans based on the individual’s specific needs, recovery goals, and medical requirements. We collaborate with families, therapists, and healthcare professionals to ensure the right balance of care and therapy.
Mobility and physical support: Regaining strength and mobility can be one of the most difficult parts of rehabilitation. Our carers are there every step of the way, assisting with exercises, providing support during walks, and helping with mobility aids. We work hard to encourage independence while offering the right level of assistance to keep your loved one safe and supporting them to positively progress through their recovery.
Medication management: Our carers are trained in medications management. They ensure medications are taken on time, prescriptions are available when needed and medication regimes are closely monitored and managed, with oversight from a Care Consultant local to you.
Sensitive personal care: During the rehabilitation process, individuals may need assistance with personal care, such as bathing, dressing and grooming. Our carers provide dignified, respectful support to ensure comfort and well-being while promoting independence wherever possible.
Preparation of meals: Proper nutrition plays a vital role in the recovery process. Our carers assist with meal planning and preparation, ensuring that meals are nutritious, balanced, and in line with any dietary restrictions, whilst being mindful of preferences.
Emotional support: Rehabilitation can be physically and emotionally upsetting. Our carers provide much-needed companionship, offering encouragement and reassurance throughout the recovery journey. This emotional support helps reduce feelings of isolation and anxiety, promoting a positive mindset during the healing process.
Household tasks: Daily chores can become difficult during recovery, which is why our carers help with light housekeeping such as cleaning, laundry, and grocery shopping. This allows individuals to focus entirely on their rehabilitation while ensuring their home remains a safe and comfortable environment.
Benefits of rehabilitation care at home
Recovering at home offers several significant advantages over rehabilitation in a clinical setting. Some of the key benefits include:
Familiar environment: Home is where people feel safest and most comfortable. Being surrounded by familiar surroundings, loved ones, and personal belongings can promote emotional well-being and aid a positive recovery process.
Personalised care: With one-on-one attention from a dedicated live-in carer or nurse, rehabilitation care at home is tailored to the individual's specific needs, goals, and preferences. This personalised approach can help speed up recovery and improve overall outcomes.
Increased independence: Rehabilitation at home encourages individuals to practise and regain daily living skills in their natural environment, making the transition to full independence smoother and more effective.
Flexibility and convenience: At-home care allows for greater flexibility in terms of scheduling therapy sessions and treatments. It also eliminates the need for travel to and from healthcare facilities, which can be physically exhausting for those in recovery.
Emotional support: Rehabilitation can be a demanding process. Having a carer present to offer not just physical support, but also emotional reassurance can make a significant difference in motivation and recovery outcomes.
Who is rehabilitation care at home for?
Rehabilitation care at home is designed for anyone who is recovering from illness, injury, surgery, or managing a long-term condition that impacts the activities of daily life. Whether the goal is to regain mobility, improve cognitive function, or improve independence, rehabilitation care can make a significant difference in the recovery process. Here are some of the individuals who can benefit from rehabilitation care at home:
Post-surgery patients: After a major surgery, such as joint replacements, cardiac surgery, or abdominal procedures, patients often need support during recovery. Rehabilitation care at home ensures they can heal safely, while receiving help with mobility, pain management, wound care, and daily activities.
Stroke survivors: Rehabilitation is a crucial part of stroke recovery. Individuals who have suffered a stroke may need assistance with physical therapy, speech therapy, and cognitive exercises. Our carers provide the on-going support needed to regain independence and adapt to any new limitations in a familiar environment.
People living with neurological conditions: Those living with conditions like Parkinson’s Disease, Motor Neurone Disease, or have incurred traumatic brain injuries can benefit from rehabilitation care at home. Our carers help manage symptoms, assist with daily routines, and support with physical and cognitive rehabilitation exercises.
Individuals with orthopaedic injuries: Fractures, joint replacements, or serious sprains can limit mobility and make daily activities difficult. Rehabilitation care provides the necessary assistance to help individuals regain strength, mobility, and coordination while preventing complications like muscle atrophy.
Heart disease and cardiac recovery: Individuals recovering from a heart attack, heart surgery, or managing chronic heart disease can benefit from rehabilitation care. Our carers provide support in medication management, encouraging safe physical activity, and making lifestyle changes that promote heart health.
People recovering from illness or injury: Whether recovering from a prolonged illness, an accident, or a sudden injury, rehabilitation care at home helps individuals regain strength and return to normal life. This can include assistance with mobility, rebuilding endurance, and gradually resuming everyday activities.
How we provide rehabilitation care at home
At Consultus Care and Nursing, we offer two distinct live-in care services for those requiring rehabilitation at home. We are committed to ensuring our clients receive the right level of care that meets their needs, whilst providing choice as to how their care arrangement is managed.
Managed Live-in Care: Our fully managed live-in care service is regulated by the Care Quality Commission (CQC) in England, which results in regular inspections to ensure the care we provide is of the highest standard. We fully manage the service, which means carers are supervised, managed and monitored by us when working in a client’s home. An expert Care Consultant local to you will plan the care you receive, regularly reviewing the service to ensure it continues to meet evolving needs. Our in-house team is there 24 hours a day to provide support to clients and carers. Peace of mind, just when you need it most.
Live-In Nursing: Our live-in nurses are fully qualified registered nurses, with a minimum of three years post qualification experience. They are carefully selected for their specific skills and knowledge and have a broad range of experience including community and acute nursing, and palliative care. A live-in nurse can provide the next level of care needed for individuals with more complex healthcare needs associated with their rehabilitation. Our service means you can avoid a prolonged stay in hospital As with our managed live-in care service, our nursing service is regulated by the CQC, and we provide 24/7 support to our clients, their families and our nurses so everyone has the support they need.
