Client Log Book to aid in continuity of care


Consultus has developed a Client Log Book in response to feedback from our clients and carers. It is hoped that the Log Book will promote continuity between carers, the client and their family.

The client, their family and carers can use the Log Book to record vital information about the client and their household, such as emergency contacts, medication, daily routines and client history. The book is designed to be updated daily, to ensure the information remains accurate and to enable as seamless changeover as possible between carers.
The Client Log Book is invaluable in providing a swift and comprehensive introduction between client and carer.
Used correctly, the Client Log Book will aid in the successful delivery of care and encourage a positive working relationship as quickly as possible.

Significant changes in the client’s care needs must still be flagged with Consultus’ Care Consultants to ensure our records are updated as quickly as possible.

Existing and new clients will receive a Log Book in the post from the end of July.
